Entry Requirements: *LOCAL Beautiful Miss US Model Title & STATE Miss Beautiful US Model Title Entry Form Entry Fee: 30 PE Bucks **Please submit a head shot and your PE Bucks via email to:
WHO WINS THE OVERALL TITLE? A contestant can better his or her chance of winning by entering all of the optional categories and submitting multiple entries for each category. The fee for each entry is the same. The highest score for each category submission will be factored into the contestant's overall score.

OFFICIAL PRIZES *A Beautiful Certificate of Recognition
(We reserve the right to replace the crown featured above with
one of equal or greater value. We must have at least 10 NATIONAL
contestants to award the big national crown. If we do not receive
10 national contestants, the OVERALL winner will receive a lovely
crown, certificate of recognition, a pageant tote, and a
sash pin.) **NEW** If we receive more than 15 national contestants, we will also award a $100 CASH prize!
SPONSOR PRIZES:
NOTE" Whatever happens.. even if we only get 1 contestant, the national winner will be getting an awesome crown! It will be gorgeous! Afterall, the national winner will be our promotion queen for the next contest, so she has to have an impressive crown!
Optional Prizes: *Optional titles will be listed on the mailed certificate of recognition. If 5 or more optional titles are awarded, a small queens gift will be mailed along with the certificate.
SPONSOR PRIZES MUST BE CLAIMED FROM THE SPONSORS DIRECTLY. WE CANNOT ACCEPT RESPONSIBILTY FOR THE REDEMPTION OF SPONSOR PRIZES. IF YOU HAVE PROBLEMS CLAIMING A PRIZE, PLEASE REPORT IT TO OUR OFFICES SO THAT WE CAN CONTACT THE SPONSOR AND/OR REMOVE THE PRIZE FROM OUR SPONSOR LIST.
